We currently collect and process the following information:

o Past medical history – Medical letters from health professionals
o Current medications – Repeat prescription list

How we get the personal information and why we have it
Most of the personal information we process is provided to us directly by you for one of the following reasons:
• We can contact you regarding your current condition
• We can contact you to arrange an appointment
• We can contact you to follow-up on your care
• We have a full history of your condition and anything that may be having an impact on it
• We can provide appropriate treatment

We also receive personal information indirectly, from the following sources in the following scenarios:
• If you have private medical insurance so that we can arrange treatment
• From your solicitor if you have been involved in an accident and we are being asked to arrange your treatment
• From an intermediary company who your solicitor may have instructed to arrange your treatment through us
• From another health care professional if you have asked them to send us a referral letter or update on your treatment

We use the information that you have given us in order to create a treatment plan, contact you to make arrangements for appointments, send you reminder SMS or email messages and to send you information regarding your treatment.
We may share your contact details with our call answering service to contact you to make appointment arrangements.  
We may share this information with your solicitor if a Data Subject Access Request is produced and signed by you.
We may share this information with another medical professional directly involved in your care if you ask us to write a referral letter for you.

Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:

How we store your personal information 
Your information is securely stored digitally on our practice management system – which uses a dedicated server at the Rackspace London Data Centre.
 
We keep your contact details and medical records for 10 years from the conclusion of treatment. We will then dispose your information by Practice Pal automatically deleting the records.

Your data protection rights
Under data protection law, you have rights including:
Your right of access - You have the right to ask us for copies of your personal information. 
Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete. 
Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances. 
Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances. 
Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.
Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
